About the Role:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our night shift team! As an LPN at Green Palms Health & Rehab of St. Marys, you will play a vital role in providing high-quality care to our residents. You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor resident conditions, and administer medications as needed.
Responsibilities:
Provide direct resident care, including vital signs, bath, dress, and grooming
Administer medications and treatments as prescribed by physicians
Monitor resident conditions and report any changes or concerns to the nursing staff
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Collaborate with the care team to ensure seamless transitions and quality care
Maintain accurate and up-to-date resident records
Participate in quality improvement initiatives to ensure high standards of care
Requirements:
Current LPN license in the state of Ohio
0 years of experience (new graduates welcome to apply!)
Ability to work a night shift schedule (variable hours)
Strong communication and interpersonal skills
Ability to lift, push, and pull patients and equipment
