We are offering a full-time Lead Support Coordinator position. This position plays a vital role in improving the lives of individuals with developmental or intellectual disabilities by connecting them to the supports they need to live independently and thrive in the community.

Responsibilities

This position is primarily responsible for providing oversight of the DD Wait List, determines eligibility, assist with ISP funding, assist with resources while on the wait list. The position also provides assistance and support to the program managers and Directors. This position manages a smaller caseload that should require less time and intervention due to the waiver wait list oversight.

The Lead Support Coordinator is responsible for slot assignments and same day access assessments for the DD case management program. This position is also responsible for the overall assessment of consumer's strengths and limitations and the initiation of Waiver and SPO Case management services in accordance with the local, state and federal quality assurance standards for case management.

Required Education and Experience

• Bachelor’s degree in human services field or related field.
• Minimum one (1) year position related experience working directly with individuals who have developmental disabilities.
• Meets the DBHDS definition of "Qualified Developmental Disabilities Professional” (QDDP/QDIP/ QMRP).
• All employees that provide case management services must complete the case management curriculum developed by DBHDS within 30 days of employment.
• Developmental Disabilities Support Coordinator must complete the PCP training modules developed by DBHDS within 30 days of hire.
• Valid Driver’s License with a point balance of -6 or higher.
• Required Knowledge, Skills and Abilities
• Services and systems available in the community including primary health care, support services, eligibility criteria and intake processes and generic community resources.
• The nature of serious mental illness, developmental disability, substance use disorders, or co-occurring disorders depending on the individuals served, including clinical and developmental issues.
• Different types of assessments, including functional assessment, and their uses in service planning.
• Support modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.
• Types of mental health, developmental, and substance abuse programs available in the locality.
• The service planning process and major components of a service plan.
• The use of medications in the care or support of the population served, and all applicable federal and state laws and regulations and local ordinances.
• Identifying and documenting an individual's need for resources, services, and other support.
• Using information from assessments, evaluations, observation, and interviews to develop person-centered plans.
• Identifying and documenting how resources, services, and natural supports such as family can be utilized to promote achievement of an individual's personal desired life outcomes.
• Coordinating the provision of services by diverse public and private providers.
• Work as team members, maintaining effective inter- and intra-agency working relationships.
• Work independently performing position duties under general supervision.
• Engage in and sustain ongoing relationships with individuals receiving services.

Preferred

• Three (3) years position related experience working directly with individuals who have developmental disabilities

Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection. Our Agency maintains a drug-free workplace.

Annual Salary Range

Grade S-C7

$52,576.00 - $59,148.00
