Greater Outreach Services, Inc. is seeking an energetic, community-focused Outreach Coordinator with a proven ability to generate referrals, establish community partnerships, and connect individuals with behavioral health and substance use services.
This position is ideal for someone who already understands community outreach and can demonstrate a successful track record of developing referral relationships and connecting potential recipients to services.
Primary Responsibilities
- Conduct community outreach to increase awareness of GOS behavioral health, substance use, and support services.
- Actively develop and maintain referral relationships with community agencies, hospitals, healthcare providers, shelters, schools, DSS, court systems, probation/parole, detention and reentry programs, treatment providers, and other community partners.
- Generate consistent and appropriate referrals for GOS programs and services.
- Follow up with referral sources to maintain strong working relationships.
- Attend community events, resource fairs, networking events, meetings, and outreach activities.
- Provide education to potential recipients, families, community partners, and referral sources regarding available services and eligibility requirements.
- Occasionally work on-site at community locations to assist potential recipients with completing referral and intake information.
- Assist individuals with navigating the initial steps necessary to access services.
- Maintain accurate documentation of outreach contacts, referral activity, community partnerships, and follow-up efforts.
- Communicate referral information promptly to the GOS Intake and Utilization teams.
- Participate in scheduled outreach meetings and provide updates regarding referral activity, community contacts, and outreach efforts.
- Represent Greater Outreach Services professionally within the community.
Qualifications
- Previous experience in behavioral health, substance use services, healthcare, social services, community outreach, marketing, case management, or a related field preferred.
- Must have a proven track record of developing community relationships and generating referrals.
- Existing relationships with community organizations and referral sources are strongly preferred.
- Knowledge of behavioral health, substance use, peer support, and community-based services is preferred.
- Strong interpersonal, communication, networking, and relationship-building skills.
- Comfortable approaching organizations and individuals to establish new referral relationships.
- Ability to work independently and manage outreach activities with limited supervision.
- Ability to travel throughout the assigned service area.
- Valid driver's license and reliable transportation required.
- Ability to maintain confidentiality and follow HIPAA and agency requirements.
Schedule
This is a part-time position averaging approximately 10–20 hours per week. Hours may vary based on community events, outreach activities, referral opportunities, and on-site intake needs. Some flexibility in daytime and occasional evening hours may be required.
