Position Overview: The One Care LTSSC works with 21-64 years old and persons with disabilities to ensure they have access to services to allow them to live as independently as possible. As a One Care LTSSC you will meet with your consumers to assess their needs, educate them about available services and assist them in securing the services they need.
Position Responsibilities: Complete initial and ongoing assessments of the health and functional status of members. These assessments are done in person in the member's home whenever possible. Manage a large diverse caseload, focusing on the members social determinants of health needs, person-centered care, support with problem solving and overall health & wellness coaching.
Qualifications
- Minimum of an Associate's Degree in Counseling, Human Services, or a related field, Bachelor's Degree preferred
- Minimum of two (2) years' experience providing care management in a health and/or human services organization, providing services to adults of all ages and disabilities is required
- Bilingual in Spanish preferred
- Excellent communication skills with the ability to establish positive and professional with team members, providers, consumers, and their support system.
- Proficient computer skills
- The ability to appropriately handle confidential and personal information, meeting the Agency standards for HIPAA compliance and data security
- Valid Drivers License and Reliable transportation with the ability to conduct home visits throughout the GLSS service area
Hours: Full Time- 40 hours Monday through Friday
Salary : $24 - $25
