The Social Worker Program Coordinator is independently licensed at the Advance Practice level to the Health Care for Homeless Veterans (HCHV) Contracted Residential Services (CRS). The HCHV CRS Liaison is responsible for the monitoring and coordination of services provided by community-based programs funded under the VA emergency housing contracts.
In addition to the experience above, candidates must demonstrate all of the following KSAs:

Physical Requirements: Light lifting (under 15 pounds); light carrying (under 15 pounds); reaching above shoulder; use of fingers; both hands required; walking; standing; hearing (aid permitted). Ability to operate vehicles with frequent travel into the community is required.
TRAVEL REQUIRED: Occasional by car to travel to Veterans Contract Residential Services Programs and or in the community to assist Veterans with housing as needed and also to conduct site visit of contract agencies.

Qualifications:
Basic Requirements- Citizenship: Citizen of the United States.
- Licensure: Candidate must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
- Licensure: Candidate must be licensed or certified by a state at the advanced practice level, and must be able to provide supervision for licensure.
- Education: Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for master's degree in social work.
In addition to the experience above, candidates must demonstrate all of the following KSAs:
- Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
- Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
- Ability to supervise multidisciplinary staff assigned to the program.
- Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
- Ability to provide training, orientation, and guidance within clinical practice.

Responsibilities:
- Provide supportive services and case management to Veterans residing in Contract Emergency Residential Housing facilities.
- Utilizes various treatment modalities of individual and group work, casework, community organization and community outreach activities to alleviate or remove problems that interfere with the Veteran's ability to achieve independence.
- Monitors the clinical care and asses the compliance of the programs receiving payments, as described in the recipient's contract. When necessary, the incumbent intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates and monitors plans of correction.
- Provides oversight of the Contract Residential Services as defined by VHA policy.
- Serves as the Contracting Officer Representative (COR) for the Contract Emergency Residential Housing Program, upon completion of required training and certification.
- Coordinates care and acts as an advocate on behalf of Veterans to ensure comprehensive service delivery and provides linkages for Veterans to VHA health care services, Veterans Benefits Administration (VBA) and community agencies.
- Provide a wide range of services, including assessments, referrals, short-term case
management, advocacy, and service coordination. - Provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate.
- Provide consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on healthcare, adherence to treatment, and housing plans.
- Participate in conducting team conferences to promote professional development, training, and coordination of service delivery.
