Job Overview
Gratitude Adjustment Integrated Healthcare is seeking an experienced Physician (MD/DO), or Family Nurse Practitioner (FNP) to join our growing team in Nancy, Kentucky. This provider will offer primary care family medicine services within an integrated healthcare setting in a rural community that includes behavioral health, medication-assisted treatment, and holistic wellness services.
We are looking for a compassionate and experienced provider who values patient-centered care, collaboration, and community impact.
This is an excellent opportunity for a provider seeking part-time work in a supportive, multidisciplinary environment. This position is full-time for an FNP candidate who holds independent licensure with national PMHNP board certification and willing to provide services in both capacities.
Duties
- Provide comprehensive family medicine services to patients of all ages
- Conduct physical exams, diagnoses, and treatment planning
- Prescribe medications and manage ongoing care
- Collaborate with behavioral health providers and care team members
- Maintain accurate and timely documentation within the EMR
- Participate in integrated care coordination when appropriate
Qualifications:
- MD/DO, or Nurse Practitioner (NP) licensed in Kentucky
- Board Certified or Board Eligible (for MD/DO)
- Active DEA license
- Prior family medicine or primary care experience required
- Strong communication and patient engagement skills
- Ability to work independently in a collaborative environment
Preferred Experience:
- Integrated care or behavioral health settings
- Medication-assisted treatment (MAT)
- Rural healthcare experience
Schedule:
- Part-time
- Full-time if FNP and board certified PMHNP
