Job Overview

We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to join our team during overnight shifts. Responsibilities

• Perform thorough patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
• Administer medications and injections accurately, including IV infusions and catheter care, following established protocols and safety standards.
• Monitor patients’ conditions continuously, recognizing signs of acute pain, infection, or deterioration, and escalate concerns promptly to supervising physicians or registered nurses.
• Assist with complex procedures such as tube feeding, venipuncture, dressing changes, and sterile processing techniques while maintaining aseptic technique.
• Document all patient interactions meticulously in medical records, ensuring compliance with HIPAA regulations and long-term care documentation standards.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for residents with diverse needs including dementia care, Alzheimer's care, or behavioral health challenges.
• Support discharge planning and coordinate with family members or caregivers to ensure continuity of care in post-acute or home settings.

Requirements

• Valid license as a Licensed Practical Nurse (LPN) in good standing with the state licensing board.
• Proven experience working with geriatric populations in settings such as skilled nursing facilities, assisted living communities, or long-term care environments.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and infection control protocols.
• Experience with EMR/EHR systems such as Epic, Cerner, or Athenahealth; familiarity with medical imaging or laboratory experience is a plus.
• Ability to perform physical examinations and vital signs assessments accurately; proficiency in CPR/BLS certification is required.
• Demonstrated competence in medication administration—including injections—and IV insertion/infusion techniques.
• Excellent communication skills for effective caregiver communication and patient assessment; compassionate approach to working with individuals with disabilities or developmental delays.
