Responsibilities-

This position is responsible for ensuring that residents are linked to the supportive services they need to successfully discharge from Grand Village. This position includes providing general case management, including care conferences and referral services to all residents needing such assistance.

 Essential job responsibilities include: 

1. Establishes linkages with community agencies and service providers to ensure individualized, flexible and creative services for the residents.

1. Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual resident.

1. Documents activities and provides required reporting, and assists with providing information required for billing. 

1. Establishes and maintains a directory of providers for use by staff, residents and residents’ family members.

1. Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate and referring residents to service providers in the community. Helps residents build informal support networks with other residents, family, and friends.
2. Scheduling and attending care conferences with residents, families and interdisciplinary team.

Qualifications

• Ability to communicate effectively in both verbal and written formats
• Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
• Bachelor's Degree in Social Work and licensed as a Social Worker or qualified experience meeting the needs of this department as determined by the Social Services Manager.
