Job Summary: As an RN Case Manager, you will be responsible for the comprehensive management of new admissions, ensuring a smooth transition from admission to discharge. You will play a crucial role in coordinating care, communicating with multidisciplinary teams, and supporting residents and their families throughout their stay.
Key Responsibilities:
- Admission Processes: Complete all necessary paperwork and initial assessments for new residents, including Level of Care determinations.
- Discharge Planning: Coordinate safe and effective discharge plans by collaborating with doctors, nurses, therapists, and external agencies like Hospice.
- Follow-up Care: Conduct follow-up calls post-discharge and collect data for quality assurance.
- OBRA Services: Monitor and organize OBRA forms monthly, ensuring compliance with recommendations.
- Communication: Attend meetings to discuss admissions and discharges, and participate in care conferences for residents.
- Certification Support: Assist the ADON in meeting the requirements for Post-Acute Care Certification.
- Appointment Management: Schedule and review appointments with residents to ensure continuity of care.
- Other Duties: Perform additional tasks as assigned to support the rehab unit's operations.
- Education: Graduate from an accredited school of nursing.
- Licensure: Valid and current R.N. license in the state of Michigan.
- Experience: Minimum of two years of related experience, preferably in geriatric nursing.
- Skills: Strong interpersonal skills, knowledge of infection control and quality assurance activities, and the ability to establish effective working relationships.
- Physical Requirements: Ability to stand, walk, lift, bend, and sit as required in a busy healthcare environment.
- Exposure to noise, frequent interruptions, and unpleasant odors.
- Compliance with the organization’s dress code.
- Utilization of potentially dangerous equipment.
