Direct Support Professional/Caregiver

GraceAge Cares is a licensed Home and Community-Based Services (HCBS) provider dedicated to supporting individuals with intellectual and developmental disabilities (IDD), seniors, and those with physical challenges. Our mission is to deliver compassionate, respectful, and person-centered care that promotes independence and quality of life.

We are seeking compassionate and dedicated Direct Support Caregivers to provide essential care and support to individuals in need. As a Direct Support Caregiver, you will play a vital role in enhancing the quality of life for clients by assisting them with daily activities and promoting their independence. This position requires a strong commitment to patient care, empathy, and the ability to work effectively in various settings, including home care, community care, and nursing homes.

Responsibilities

• Provide personal care assistance, including bathing, grooming, dressing, and toileting.
• Support clients with mobility and lifting as needed to ensure their safety and comfort.
• Assist with meal preparation and feeding when necessary.
• Engage clients in meaningful activities that promote social interaction and mental stimulation.
• Monitor clients' health status and report any changes or concerns to the appropriate healthcare professionals.
• Maintain a clean and safe living environment for clients.
• Ensure compliance with HIPAA regulations and maintain confidentiality of client information.
• Provide companionship and emotional support to enhance clients' well-being.
• Document care provided accurately in accordance with company policies.
• Follow individual service plans (ISPs) and adhere to company and DDA/DIDD policies.
• Document services and progress notes accurately and timely, in accordance with state regulations.

Requirements

• Previous experience in caregiving or related fields such as home community care, hospice care, or dementia care is preferred.
• Strong understanding of patient care principles and practices.
• Ability to lift clients safely and assist them with mobility challenges.
• Excellent communication skills and the ability to build rapport with clients.
• Knowledge of HIPAA regulations regarding patient privacy is essential.
• Compassionate nature with a genuine desire to help others improve their quality of life.
• Flexibility to work various shifts as needed, including evenings and weekends.
• Certification in CPR/First Aid is a plus but not mandatory.
• Must be at least 18 years of age.
• High school diploma or GED required.
• Valid driver's license and reliable transportation preferred.
• Must be able to pass a fingerprint-based background check through TBI/IdentoGO.
