Job Type: Part-time, flexible hours (Hours will change week to week.) Hours are based on when we have a new client assessment and every 6 month reassessments based on when client signs up. Also occasional training of CNAs. (Approx. 12 hrs. a month.)

Your Role at Grace Care:
As a vital part of our leadership and care team, you will:
- Conduct initial nursing assessments and develop personalized care plans for new clients (in partnership with the Care Manager)
- Accurately enter client information into AxisCare (we’ll train you if needed!)
- Perform client reassessments every 6 months
- Serve as an on-call consultant for clinical questions or concerns
- Offer CNA training and ongoing education, as needed
- Participate in quarterly staff meetings and leadership team discussions
- Guide, support, and inspire caregiving staff with professionalism and care
- Help ensure our mission of quality, dignity-driven service is upheld every day
What We’re Looking For:
- Active RN license in Oklahoma
- Experience in home care or similar healthcare setting is a big plus
- Strong communication, time-management, and organizational skills
- Tech-comfortable or willing to learn (especially AxisCare)
- Passion for mentoring caregivers and sharing your clinical knowledge
- Flexible, dependable, and aligned with our values of dignity and compassion
