Residential Treatment Home (RTH)

Grace Care Homes is seeking compassionate and dependable Direct Care Staff to support adults experiencing Severe and Persistent Mental Illness (SPMI) in a residential treatment setting.
Our mission is to empower minds, restore hope, and help individuals build the skills needed to live meaningful and independent lives.
Responsibilities
- Provide direct care and supervision to residents.
- Assist with activities of daily living (ADLs) and independent living skills.
- Administer and document medications as prescribed.
- Complete daily charting, progress notes, and incident reports.
- Support residents with appointments, community activities, and personal goals.
- Prepare meals and perform light housekeeping.
- Respond to emergencies and utilize de-escalation techniques when needed.
- Maintain compliance with Oregon Administrative Rules, Resident Bill of Rights and Home and Community-Based Services.
Qualifications
- High School Diploma or GED.
- Must be at least 18 years old.
- Ability to pass a criminal background check.
- Strong communication and interpersonal skills.
- One year experience in caregiving, behavioral health, or mental health services preferred.
Schedule
- Saturday 7 AM - 7 PM & Sunday 7 PM - 7 AM
