Role Description
This is a full-time remote role for a Community Liaison. The Community Liaison will be responsible for building and maintaining strong relationships with community partners, conducting outreach programs, and serving as a vital connection between the organization and the communities it serves. Daily tasks include representing the hospice at events, fostering community engagement, providing exceptional communication and customer service to patients and families, and collaborating with the care team to ensure seamless support for home care services.
Qualifications
- Strong skills in Community Engagement and Community Outreach
- Excellent Communication and Customer Service abilities
- Experience with Home Care services and a commitment to compassionate care
- Ability to build and maintain partnerships with community organizations
- Strong organizational and time management skills
- Familiarity with hospice or healthcare services is a plus
- Driver’s license and reliable transportation (if required for community outreach activities)
- Bachelor’s degree in Social Work, Public Health, Communications, or a related field preferred
