Job Summary
In this entry-level case management role, you will be responsible for assisting individuals and families in accessing the health, social, and community-based services they need to thrive. You will serve as a trusted advocate, connecting clients to the appropriate programs and resources, while also providing ongoing support and guidance throughout their journey.

Key Responsibilities

• Conduct comprehensive assessments to identify clients' needs and develop personalized service plans
• Coordinate with a variety of community partners and service providers to facilitate access to essential services
• Maintain detailed case records and documentation to ensure efficient and effective service delivery
• Advocate on behalf of clients to ensure their needs are met and their rights are protected
• Provide emotional support and guidance to clients, empowering them to achieve their goals
• Participate in team meetings and collaborate with colleagues to identify and implement process improvements

Qualifications

• Bachelor's degree in social work, human services, or a related field
• Experience in case management or direct service delivery preferred
• Strong communication and interpersonal skills, with the ability to build rapport and trust with diverse clients
• Demonstrated problem-solving skills and a commitment to providing high-quality, client-centered care
• Knowledge of community resources and social services programs
• Proficiency in relevant software and data management tools
