The Awesome Things You’ll Do
- Perform intensive on-the-job training with newly placed consumers.
- Train the consumers on public transportation alternatives to get to/from work.
- Provide ongoing coaching on a monthly basis for consumers needing long-term follow along (LTFA).
- Keep track of task analysis for the purpose of training new hires on the job.
- Maintain case documentation on a weekly basis for intensive job coaching consumers. Write case notes on a monthly basis for LTFA consumers.
- Track # of service hours provided to each consumer and submit activity reports on time to get vouchers for additional hours when needed.
- Perform case management services as required, to include linking the trainee with other service providers or community resources such as: recreation, education, transportation or leisure, in order to further enhance the trainee’s integration into the community.
- Train consumers’ new co-workers and supervisors on how to best work with the newly placed consumer.
- Assist to find jobs for pre-placement clients including conducting pre-placement activities, workshops and job search.
- Contacts DVRS & other referral agencies for placement, supported employment and/ or job coaching referrals. Submit monthly job coaching reports. Participate in CARF audits.
- Attend rehab dept and all staff meetings. Attend job development meetings.
Our Must Haves
- AA Degree in human services field from an accredited college. Alternatively, High School diploma and 3-5 years’ experience in related field.
- Knowledge of MS Word, Excel and Outlook required. For job coaches working with deaf and HOH consumers, fluency in American Sign Language required and SCPI certification preferred.
- Must have a valid NJ driver’s license and pass license check. Must be willing to use private transportation to travel to employers and serve consumers in counties throughout Northern NJ.
- The Job Coach reports to the Director of Rehabilitation and does not supervise other staff.
- Job primarily consists of field on-site and offsite face to face visits with consumers.
- Expected to make site visits during consumer work schedules and will consist of night and weekend work in the community.
