Hospital-Based Hospice Nurse Liaison
Named by the Boston Globe as one of the Top Places to Work in Massachusetts for the last four years, our community based, mission-driven hospice and palliative care organization seeks a Hospital-Based Hospice Nurse Liaison to join its expanding Intake and Admissions team. This is an on-site position located at Newton-Wellesley Hospital.
Job Responsibilities:
The position of Hospital-Based Hospice Nurse Liaison (HBHNL) is to provide information about hospice services to patients, families, and hospital-based staff, to conduct clinical assessments, coordinate hospital discharge plans, and maintain professional relationships within hospitals, with a focus on the assigned hospital.
Schedule and Weekly Hours:
Monday through Friday from 9 am to 5 pm (40 hours)
Education, Skills and Experience:
- Licensed by the Commonwealth of MA as a Registered Nurse.
- At least two years experience preferred in community health, oncology, hospice, or palliative care.
- Excellent communication and interpersonal skills
- Ability to work collaboratively with hospital staff, Good Shepherd colleagues, patients, and their families
- Highly skilled at providing patient assessments and evaluations.
- Excellent problem-solving skills
- Competency with accepted standards of hospice nursing practice.

Pay Range:
$89,000 - $106,000: This number reflects an average starting range for this position. Individual pay is determined through a variety of factors, including experience, education, certifications, and skills and can exceed the starting range.
