Good Shepherd Fairview Home is looking for a Physical Therapist to join our exceptional Outpatient Therapy team!
Full Time - 30 hours (up to 40 hours) per week
Position Summary:
We are seeking a compassionate and skilled Outpatient Physical Therapist to join our team. This therapist will work across all levels of care, including skilled nursing, assisted living, and independent living, but will primarily focus on outpatient therapy services. This role is ideal for a motivated clinician passionate about improving the lives of geriatric patients through evidence-based interventions.

Essential Job Duties and Responsibilities:
The PT is responsible for the screening of residents of the facility as follows:
- Provide comprehensive outpatient physical therapy services to residents and community members.
- Evaluate, diagnose, and develop individualized treatment plans to improve mobility, strength, and functional independence
- New facility residents – within 24 hours of admission
- Long Term residents – upon receipt of a significant change form and/or referral from nursing

The PT is responsible for the completion of all necessary documentation, which includes but is not limited to the following items: initial evaluation forms, weekly progress notes, monthly recertification forms and discharge summary.
Essential Job Requirements
Education: Doctoral Degree in PT, Masters of Science Degree or Bachelor of Science Degree in Physical Therapy.
License: State Licensed Physical Therapist.
Qualifications: Must be 18 years of age. Ability to read, write and speak the English language
Experience: Preferred working in a long term care facility.
