Job Summary:
Join our team at Goldwater Care Pontiac, a boutique organization committed to clinical excellence and compassionate service, as a Physical Therapist in an in-house skilled nursing facility. This full-time hourly position offers a 40-hour workweek with a Monday-Friday schedule, no weekends required, and flexible start/end times. As a Physical Therapist, you will be responsible for evaluating and treating patients to achieve the highest level of functional outcomes and patient satisfaction. On-site training and guidance will ensure your success in this role.
Essential Duties and Responsibilities:
Provide individualized Physical Therapy services based on patients' unique clinical needs with courtesy and respect.
Evaluate and treat patients with a wide range of medical conditions.
Manage a daily caseload of 8-10 patients for a regular 8-hour day.
Supervise a full-time Physical Therapy Assistant and PRN therapists as needed.
Complete necessary documentation and billing in the EMR for therapy services.
Participate in administrative tasks, such as insurance updates and durable medical equipment paperwork.
Conduct screens for therapy services to ensure long-term care resident needs are met.
Provide educational in-service trainings to nursing staff.
Collaborate with the therapy team to provide high-quality care.
Advocate for patients and communicate with families with compassion.
Actively participate in therapy team and facility meetings.
Qualifications:
Current and in good standing licensure as a Physical Therapist in the state of IL.
Current CPR certification.
Experience in Long Term Care is preferred but not required.

Physical Demands:
Be able to move intermittently throughout the workday.
Meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.
