What You'll Own

Person-Centered Care

  • Complete comprehensive assessments and build Individualized Service Plans that meet Kentucky Medicaid requirements (907 KAR 7:010).
  • Conduct home and community visits to monitor health, safety, satisfaction, and progress toward goals.
  • Advocate for participant preferences and rights, and make sure informed choice is real, not paperwork.
  • Coordinate across Golden Years programs and outside providers — attendant care, transportation, therapies — within authorized scopes.

Regulatory Excellence

  • Maintain compliance with Kentucky Medicaid and HCB Waiver regulations, including conflict-free case management standards.
  • Complete assessments, ISPs, reviews, and progress notes within mandated timeframes.
  • Document contacts, visits, coordination, and critical incidents so we are always audit-ready.
  • Review service utilization and correct under- or over-utilization before it affects care.
  • Contribute to quality improvement and corrective action work.

What You Bring

Required — Non-Negotiable

Prior hands-on experience in case management, care coordination, or waiver services is required. This is not a training role and it is not negotiable.

Participants and families depend on a case manager who can carry a caseload from day one.

  • Documented experience in case management, care coordination, or waiver services. Required, no exceptions.
  • Working knowledge of person-centered planning and conflict-free case management.
  • Strong assessment, planning, and problem-solving judgment — balancing choice, safety, and regulation.
  • Clear written and verbal communication for documentation and stakeholder work.
  • Organized time management across a caseload and hard deadlines.
  • Valid driver's license, reliable transportation, and current auto insurance. Local travel is required.

Preferred

  • Bachelor's degree in social work, nursing, human services, or a related field.
  • Kentucky Medicaid / HCB Waiver experience and familiarity with 907 KAR 7:010.
  • Electronic health record experience.
  • Cultural competence working with diverse rural and urban populations.

Pay and Benefits

Base Pay: $65,000 – $70,000 per year, based on waiver experience and credentials. We pay at the top of this range for proven Kentucky waiver case management experience.

Where You'll Work

Base Office: Golden Years ADHC, 903A West Stockton Street, Edmonton, Kentucky 42129

Schedule: Full-time weekdays, with flexibility around participant visits.

Service Area: Metcalfe County and the surrounding south-central Kentucky communities.

We give preference to candidates living in Metcalfe County or a bordering county — Barren, Green, Hart, Adair, Cumberland, or Monroe. Local knowledge and short drive times mean faster response and stronger relationships.
