Responsibilities:
- Dispense medications accurately and efficiently
- Provide patient counseling on medication usage and potential side effects
- Collaborate with healthcare professionals to develop comprehensive medication therapy plans
- Conduct patient assessments to identify drug-related problems and optimize therapy outcomes
- Perform medication reconciliation to ensure accuracy and safety
- Monitor patients' response to therapy and adjust medication regimens as needed
- Participate in pharmacy quality improvement initiatives
- Provide education and training to pharmacy staff, healthcare professionals, and patients
Experience:
- Minimum of 2 years of experience as a licensed pharmacist
- Proficiency in IV infusion and medication administration
- Strong knowledge of pharmacy laws, regulations, and best practices
- Experience in a variety of pharmacy settings, such as hospital, retail, or long-term care
- Completion of a pharmacy residency program is preferred
- Familiarity with utilization review processes and formulary management
