Summary

The Case Manager is a leadership position within the Community Residence partnering with the Community Residence Program Manager, Senior Counselors, Social Worker, and Nurse. As a collective, the co-leadership team is responsible for the oversight of the health, wellness, and care of youth in the program. Knowledge of childhood trauma and the impacts is paramount. The Case Manager will identify and assess client and family needs, coordinate, and provide necessary services and/or treatment are provided. They will also provide in-home supportive services to clients, complete required assessments, and assist individuals and families by helping to articulate goals and providing needed information.

Primary Job Functions

Provide Behavioral Health Overlay Services (BHOS) interventions under the supervision of a licensed clinician while upholding the Glove House Standard of Care.

  • Guided by organizational values and trauma informed practice in all interactions.
  • Demonstrates understanding of the four dimensions of safety and can identify gaps.
  • Treats all children, families, staff and ancillary providers with respect and courtesy.
  • Demonstrates and models sensitivity to the cultural background of children, families, and co-workers.
  • Assists in ensuring that job-related activities follow Glove House policies and procedures, OMH relevant professional association, ethical standards, accreditation standards, and the law.
  • Requires daily interaction with staff and youth ensuring services are delivered in accordance with the Glove House operating modality (TCI), youth’s treatment plan, calendars and schedules are followed.
  • Assists in providing staff and resident counseling and support in crisis situations, including post-crisis response.
  • Model positive behaviors and implement core values.
  • Assists with the intake and pre-placement process.
  • Coordinate services between the Community Residence and clinic treatment programs or other community resources utilized by the residents (i.e., schools, vocational, therapy.)
  • Develop relationships with community organizations and resources to improve service access for residents in the program.
  • Maintain frequent contact with the children’s families and guardians; arrange visits with families and/or siblings; provide services to the child and family and prepare the family for reunification.
  • Coordinate and arrange transportation for home visits.
  • Ensure eligibility and subsequent payment from SSI, Medicaid, and other third-party funding sources.
  • Facilitate psycho-educational classes/groups.
  • Train direct care team members regarding components of the individualized treatment plan.
  • Participate in treatment team meetings.
  • Completes administrative tasks assigned.
  • Assists to ensure adequate staff coverage is available and assumes added responsibility for coverage when needed.
  • Ensure, complete, and maintain client records and program documentation (self, staff, and youth), such as daily activities, treatment plan progress, progress notes and termination summaries, according to Glove House program and regulatory standards.
  • Assures all documentation is completed in a timely fashion (myEvolv, etc.)
  • Will actively participate as a team member and attend staff meetings.
  • Participate in all Glove House strategic planning goals, including risk management and Quality Assurance plans.
  • Required to safely travel on agency business, as required by job demands. Maintain accurate mileage documentation and submit in required agency timeframes.
  • Consistently attends mandated agency and department meetings and participates in standing agency committees and community committees as assigned.
  • Performs other responsibilities as assigned by the Community Residence Program Manager.

Job Qualifications

Education, Licenses and Certifications

  • Bachelor’s degree in the human services field; master’s degree in the human services field preferred.
  • Must have a valid driver’s license and driving record in accordance with agency standards.
  • Minimum experience 2 years of working with children and families.

Schedule Expectations

The incumbent will work a standard scheduled week, which may include weekends, holidays and evenings. The ability to work flexible hours is required, as is the ability to provide on-call support when scheduled or to meet business needs. Travel will be as needed to conduct Glove House business-related activities. Generally, duties of this position require 80% time at work (20% is eligible as work from home time). Not all duties will occur within the program (ie. There are home visits, groups, education that occur outside of the program).
