Responsibilities
- Assist clients with activities of daily living (ADLs), including personal care, grooming, and hygiene
- Support residents with meal preparation, feeding, and nutritional needs while adhering to dietary plans
- Monitor patient health by checking vital signs, observing behavior, and reporting changes promptly
- Administer medication following prescribed care plans and ensure compliance with HIPAA regulations for patient privacy
- Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility
- Maintain a clean and safe environment through regular cleaning duties and sanitation practices
- Document care activities accurately in resident records and update individualized care plans regularly
- Provide companionship and emotional support to promote mental well-being
- Assist with transportation for appointments or community outings using personal transportation assistance methods
Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health settings
- Knowledge of patient observation techniques, dementia care including Alzheimer’s and memory care strategies
- Familiarity with medication administration, vital signs monitoring, and basic first aid/CPR certification
- Ability to perform heavy lifting safely using proper manual handling techniques and patient transfer methods
- Strong communication skills for effective client interaction and team collaboration
- Experience working with individuals with autism or neurodevelopmental disorders is highly valued
- Understanding of resident’s rights, HIPAA compliance, and confidentiality standards
- Skilled in assisting with ADLs such as bathing, dressing, grooming, and personal hygiene tasks
- Ability to handle emergency situations calmly while providing hospice or palliative care when needed
- Competence in meal preparation aligned with dietary restrictions or special needs of clients
- Knowledge of home & community care practices to support clients outside of residential settings
