We are expanding our clinical team and seeking an Admission Nurse dedicated to ensuring a smooth, thorough, and compassionate intake experience for every new patient.
The Admission Nurse (RN) is responsible for completing comprehensive initial assessments for new clients entering home health or personal care services. This role ensures accurate clinical evaluation, regulatory compliance, and a welcoming onboarding process for patients and their families. The ideal candidate is detail-oriented, highly organized, and skilled at building trust and rapport during a patient’s first point of care.
- Conduct initial nursing assessments, including physical exams, medical histories, functional status evaluations, and medication reviews
- Develop and document initial care plans in accordance with physician orders and agency standards
- Determine patient eligibility and care needs based on clinical findings and regulatory requirements
- Educate patients and families regarding care expectations, safety procedures, and available services
- Ensure all admission documentation is accurate, complete, and submitted within required timeframes
- Communicate with physicians to obtain required orders, clarifications, or adjustments
- Coordinate with the Director of Nursing, caregivers, and interdisciplinary team members for seamless onboarding
- Identify safety risks, urgent needs, and priority interventions during assessment
- Maintain compliance with HIPAA, OSHA, and state home care guidelines
- Support agency quality initiatives and assist with QAPI activities when needed
- Provide a patient-centered, compassionate, reassuring experience during the admission process
- Current Registered Nurse (RN) license in Rhode Island
- Experience in home health, home care, hospital admissions, or skilled nursing preferred
- Strong clinical assessment, communication, and critical-thinking abilities
- Knowledge of state and federal regulations for home health admissions
- Exceptional documentation and organizational skills
- Ability to work independently in the field and manage multiple admissions
- CPR/BLS certification required
- Reliable, empathetic, and committed to delivering exceptional patient-centered care
