Position Summary

The Coordinated Family Support (CFS) Support Coordinator provides individualized coordination, advocacy, and resource linkage services to adults with developmental disabilities and their families through California’s Coordinated Family Support program. The Support Coordinator works collaboratively with clients, families, Regional Centers, community agencies, and interdisciplinary teams to increase independence, stability, access to resources, and overall quality of life.

This role focuses on reducing barriers, coordinating services, promoting self-determination, and supporting families in navigating complex systems including benefits, healthcare, behavioral health, transportation, housing, vocational services, and community resources.

Essential Duties & Responsibilities

  • Particapate in intake assessments and assist in identifying client and family needs.
  • Follow individualized monthly plans with measurable goals and outcomes.
  • Maintain client encounters per week, as assigned.
  • Coordinate and monitor services and supports to ensure follow-through and continuity of care.
  • Assist families with accessing generic and community resources including:
    • IHSS
    • SSI/SSA benefits
    • Medi-Cal
    • CalFresh
    • Behavioral health services
    • Dental and medical services
    • Transportation resources
    • Vocational and educational supports
  • Support families with referrals to legal and financial planning resources, including conservatorship education, Special Needs Trusts, and CalABLE accounts when appropriate.
  • Provide advocacy and support during appointments, meetings, and service coordination processes.
  • Maintain professional communication with Regional Center staff, families, and community partners.
  • Complete service notes and documentation after  of each encounter.
  • Ensure documentation reflects measurable progress, outcomes, barriers, and next steps.
  • Participate in weekly staff meetings, trainings, supervision, and quality assurance activities.
  • Maintain confidentiality and comply with HIPAA, Title 22, DDS, and Regional Center requirements.
  • Report concerns, incidents, or barriers to supervisors in a timely manner.
Qualifications

  • Bachelor’s degree preferred in social work, psychology, human services, nursing, or related field; relevant experience may be considered in lieu of education.
  • Experience working with individuals with developmental disabilities and/or families preferred.
  • Knowledge of Regional Center services, IHSS, SSI, Medi-Cal, and community resources preferred.
  • Strong organizational, communication, and documentation skills.
  • Ability to work independently while maintaining consistent communication with supervisors and team members.
  • Ability to manage multiple cases and meet deadlines.
  • Compassionate, professional, and family-centered approach.
Requirements

  • Valid driver’s license and reliable transportation.
  • Ability to travel within assigned service areas.
  • Basic computer and documentation skills.
  • Ability to use electronic systems including email, Zoom, and documentation platforms.
  • Must pass background clearance requirements as applicable.
Work Expectations

  • Documentation must be completed after each encounter .
  • Staff are expected to maintain professionalism with families, Regional Center staff, and interdisciplinary teams.
  • Weekly supervision and staff meetings are required.
  • Productivity and service delivery must align with authorized service hours and program expectations.
