JOB TITLE:   CERTIFIED NURSING ASSISTANT 

DEPARTMENT:  LONG TERM CARE

HOURS & SHIFT REQUIREMENTS: Full time, nights (6 p – 6 a), incentive plan available.   

GENERAL SUMMARY

Under the general direction and supervision of long term care R.N. or L.P.N. the long term care C.N.A. provides personal care and assistance to residents with activities of daily living. This care is provided in accordance with a plan of care that is prescribed by the physician and written by an interdisciplinary team.

PRINCIPLE DUTIES AND RESPONSIBILITIES

1.    Collects data regarding resident care needs and response to nursing care provided.

2.    Reviews the care plan with RN/LPN and identifies potential changes.

3.    Evaluates the effectiveness of care provided and reports to the primary 
RN/LPN.

4.    Demonstrates competency in selected psychomotor skills.

5.    Implements appropriate measures to meet the resident/SO learning needs.

6.    Performs documentation duties in the resident record which is timely, accurate, and concise.

7.    Demonstrates awareness and sensitivity to residents/SO rights, as identified by the Patient Bill of Rights.

8.    Functions with an awareness and application of safety and infection control issues as identified in the facility.

9.    Performs efficiently in emergency resident situations following established protocols, remaining calm, informing appropriate persons, and documenting events.

10.    Demonstrates sound knowledge base and actions in care and decision making for designated resident population.

11.    Demonstrates an awareness of self-responsibility and accountability for own skills and actions within scope of facility’s policies and procedures.

12.    Participates actively in staff development and orientation activities for the unit.

13.    Demonstrates self-directed learning and participation of continuing education to meet own development needs.

14.    Seeks validation of knowledge base and skill level and assertively seeks guidance in areas of question. 

15.    Demonstrates awareness of legal issues in all aspects of resident care and unit functions and strives to manage situations in a reduced risk manner.

16.    Participates in development and attainment of unit and division of nursing goals.

17.    Participates in unit and division of nursing QAPI activities.

18.    Demonstrates effective communication methods and skills, using lines of authority appropriately.

19.    Uses effective working relationships with all health care team members, residents/so and staff.

20.    Remains flexible in staffing patterns and resolution of staffing conflicts: participates in temporary assignment measures of division of nursing.


PHYSICAL REQUIREMENTS

1.    Visual acuity to observe residents and use manual documentation tools.

2.    Auditory acuity to hear resident verbal communication and gather data using auscultatory skills.

3.    Manual dexterity of hands and feet to ambulate 8-12 hrs. per shift and manage multiple pieces of equipment/invasive lines safely.

4.    Must be able to follow directions.

5.    Physical strength to perform the following lifting demands:
•    Floor to waist – 40 pounds
•    14” to waist – 50 pounds
•    Waist to shoulder – 20 pounds
•    Shoulder to overhead – 10 pounds
•    Carry 40 pounds for 30 feet
•    Push 40 pounds/force for 30 feet
•    Pull 40 pounds/force for 10 feet
•    Carry 20 pounds away from body for 30 feet

6.    Keen mental faculties to observe resident status change.

7.    Communication/speaking/enunciation skills to receive and give information in person and by telephone.


8.    Emotional stability conducive to dealing with a high stress level for 8-12 hrs. at a time associated with caring for residents that may be acutely ill or possess behaviors consistent with Alzheimer’s disease, or other forms of dementia, emotional demands of residents and their families, and demands of maintaining effective working relationships with peers, managers, and other health care workers.


REPORTING RELATIONSHIP

Reports to the Long Term Care Director of Nursing and Long Term Care Administrator.
 

Qualifications:

EDUCATION, KNOWLEDGE AND ABILITIES REQUIRED

1.    Must be certified, or registered by the Illinois Department of Registration as a Nursing Assistant or have documentation of successful completion of a training course approved by another state as evidenced by a diploma or certification; be in a RN/LPN approved course, or have the potential to complete the approved orientation course and enter a CNA training course within 180 days of hire.

2.    Interested and empathetic attitude toward caring for the sick elderly, and disabled.


INFECTION EXPOSURE LEVEL

Category I– Maximal Risk - This job usually involves contact with patient or patient specimens. Exposure to blood, body fluids and tissue is possible. The Hepatitis B vaccine shall be made available.


WORKING CONDITIONS

See physical requirements. Job is highly demanding physically, and dealing with behaviors of residents with dementia has ability to cause stress.
