Job Description & Responsibilities
As an HHA, you will work under the supervision of our Director of Nursing to provide essential personal care and emotional support to our patients. Your core responsibilities will include:
- Patient Monitoring: Measure and record vital signs (pulse, temperature, respiration, and blood pressure).
- Observations: Monitor and document changes in patient behavior, physical condition, and overall progress.
- Daily Living Support: Assist patients with personal hygiene, bathing, grooming, dressing, and light mobility.
- Collaboration: Coordinate closely with our nursing and therapy teams to ensure the patient's care plan is followed accurately.
- Documentation: Complete timely, accurate reports regarding patient visits and progress.
Qualifications & Skills
- Self-Motivated: A reliable self-starter who excels at working independently in the field.
- Communication: Strong interpersonal and communication skills to connect with patients, families, and clinical staff.
- Organization: Excellent time-management and follow-up skills.
Requirements
- Active California HHA Certification (Required)
- Valid California Driver’s License and reliable transportation for travel between patient homes.
- Must be willing to maintain strict compliance with agency and state healthcare guidelines.
