Responsibilities
- Collaborate with families , caregivers , case mangers and office staff
- Staff your case load with caregivers
- Develop individualized care plans
- Bi-monthly Home Visits to your clients
- Create goals and monitor progress toward goals
- Create schedules
- communicate with the state , client , families and caregivers.
- Previous experience as a Care Coordinator or in a similar position is preferred
- Certification as a medical assistant is preferred
- Strong problem-solving and organizational skills
- Ability to manage multiple projects or tasks and prioritize appropriately
- Ability to work in fast-paced situations and make sound decisions quickly
- Excellent interpersonal skills and high level of compassion
- Strong verbal and written communication skills
- Must Drive
- Bilingual
Salary : $20 - $26
