Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and personal hygiene to promote dignity and independence.
- Monitor patients’ health status by observing vital signs, recognizing changes, and reporting findings accurately in accordance with HIPAA regulations.
- Support medication administration and adherence to care plans tailored to each individual’s needs.
- Prepare nutritious meals and assist with meal planning to ensure dietary requirements are met.
- Provide transportation assistance for medical appointments, errands, or social activities, base on clients conditions ensuring safe patient transfers using appropriate techniques like patient transfer methods.
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces.
- Document care activities meticulously, communicate effectively with clients and family members, and adhere to all home health and hospice care protocols.
Experience
- Prior experience working with individuals with disabilities, neurodevelopmental disorders such as autism or intellectual disabilities support is highly desirable.
- Experience in senior care settings such as assisted living or nursing homes enhances your ability to deliver specialized care.
- Knowledge of dementia care, Alzheimer’s care, memory care strategies, and hospice care practices is preferred.
- Familiarity with patient monitoring techniques including vital signs measurement, manual handling, patient transfer methods, CPR, first aid, and HIPAA compliance is essential.
- Proven ability to work independently within home & community care environments while maintaining professionalism and compassion.
- Experience working with diverse populations including geriatrics and individuals with developmental or neurodevelopmental disabilities will set you apart.
