Role Description The Bilingual Case Coordinator/Case Manager (Home Care) is a full-time, on-site role based in Piscataway, NJ. This role is responsible for coordinating home care services, including intake, assessment, and ongoing case management for clients and their families. Daily responsibilities include developing and updating care plans, scheduling and confirming caregiver assignments, and monitoring service quality to ensure client needs are met. The Case Coordinator/Case Manager communicates regularly with clients, caregivers, and health care providers, documenting all interactions and maintaining accurate records. The role also involves verifying insurance coverage, assisting with authorizations, and addressing client concerns or service issues in a timely, professional manner. The individual in this position helps ensure seamless, client-centered care and supports the overall efficiency of home care operations.
Qualifications
- Strong case management skills, including care coordination, client assessment, and ongoing monitoring of home care services.
- Background in social work or related human services, with the ability to advocate for clients and connect them to community resources.
- Excellent communication and customer service skills, with the ability to build trust, handle sensitive situations, and interact with diverse populations in a bilingual capacity.
- Experience working with insurance processes, including eligibility verification, authorizations, and basic understanding of home care coverage requirements.
- Fluency in English and at least one additional language commonly spoken in the community (e.g., Spanish, Polish, or another relevant language).
- Proficiency with basic office software and electronic record systems; ability to maintain accurate documentation and manage multiple cases simultaneously.
- High school diploma or equivalent required; associate or bachelor’s degree in Social Work, Human Services, Nursing, or a related field preferred.
