Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specializing in Long Term Care to join our healthcare team. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and documentation of health status.
- Administer medications and treatments accurately according to physician orders, including injections, IV infusions, and wound care.
- Monitor residents for changes in condition, promptly reporting significant findings to registered nurses or physicians.
- Assist with activities of daily living (ADLs), such as bathing, dressing, feeding, and mobility support.
- Maintain accurate medical records in electronic health record (EHR) systems while ensuring compliance with HIPAA and confidentiality policies.
- Provide education and support to residents and their families regarding health management plans.
- Collaborate with interdisciplinary teams to develop individualized care plans tailored to each resident’s needs.
- Ensure adherence to long-term care regulations, infection control protocols, and safety standards at all times.
Requirements
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Prior experience working in long-term care facilities or skilled nursing environments is highly preferred.
- Knowledge of EMR/EHR systems such as Epic or Cerner for documentation purposes.
- Proficiency in clinical skills including IV infusion, catheterization, sterile technique, and basic life support (BLS).
- Experience working with geriatric patients, individuals with dementia or Alzheimer's disease, and those requiring memory or behavioral health care.
- Strong communication skills with the ability to effectively interact with residents, families, and team members.
- Familiarity with long-term care regulations and compliance standards governing nursing home operations.
- Additional experience in hospice care, acute pain management, or trauma center environments is advantageous.
