Multi Specialty Practice servicing skilled nursing facilities has an opening for Full time /part time Nurse practitioner in the following NY location: Albany,NY
We are currently looking to fill a full-time and part time position to deliver quality care to residents in a SNF
Responsibilities:
· Management of chronic medical conditions
· Collaboration with an interdisciplinary team
· Provide high quality customer service
· Provide clinical / medical education
· Participate in care conferencing
Job Requirements:
· Nurse Practitioner with relevant clinical experience with adult pts.
· Ability to deal with acute medical situations
· Strong interpersonal, leadership and problem solving skills

Salary: $130,000-160,000 with bonus incentives, room for growth
Schedule:
· 6-8 hour shift
