Genesis Community Support Services is seeking an experienced Qualified Professional (QP) to join our team in Greensboro, NC. The ideal candidate will have previous experience functioning in a QP role and a strong working knowledge of community-based services for individuals with intellectual and developmental disabilities, including individuals who may have co-occurring mental health, behavioral health, and/or substance-use needs.
This position is responsible for the coordination, monitoring, documentation, and oversight of services for an assigned caseload. The QP works closely with individuals served, guardians, families, direct support professionals, AFL providers, care managers, LME/MCO representatives, medical and behavioral health providers, and other members of the treatment team.
We are specifically seeking a candidate who already understands the responsibilities of a QP and can assume an active caseload following agency orientation and training.
Previous hands-on experience with THERAP is required.
Duties
- Manage and provide QP oversight for an assigned caseload of individuals receiving residential, community-based, and other support services.
- Develop, implement, monitor, review, and update ISP/PCP goals and service plans in accordance with individual needs and authorization requirements.
- Monitor the delivery of services to ensure services are provided according to the individual's ISP/PCP, goals, authorized frequency, and applicable requirements.
- Complete required QP monitoring, supervision, member contacts, site visits, and follow-up within established timelines.
- Review service notes, goal documentation, case notes, health-related documentation, and other records for accuracy, quality, timeliness, and compliance.
- Maintain and monitor member records within THERAP.
- Utilize THERAP for ISP Programs, T-Logs, General Event Reports (GERs), Health Tracking, case documentation, member information, and other applicable QP functions.
- Review staff documentation and provide follow-up, coaching, or corrective guidance when documentation does not meet agency or service requirements.
- Coordinate and participate in treatment-team meetings, ISP meetings, transition meetings, discharge planning, and other interdisciplinary meetings.
- Communicate regularly with individuals served, guardians, families, care managers, LME/MCO representatives, providers, hospitals, DSS, and other members of the treatment team.
- Provide supervision and ongoing guidance to DSPs, residential staff, AFL providers, and other assigned service personnel.
- Monitor member progress toward goals and identify when changes to goals, interventions, staffing, or services should be discussed with the treatment team.
- Assist with service authorization and reauthorization processes, supporting documentation, and requests for additional information as applicable.
- Monitor significant health, behavioral, and safety events and ensure appropriate documentation, notifications, follow-up, and corrective actions are completed.
- Complete and oversee incident documentation and reporting within required timelines.
- Participate in hospital discharge and community transition planning when applicable.
- Identify service-delivery, documentation, health and safety, or compliance concerns and communicate concerns through the appropriate chain of command.
- Participate in internal audits, quality-assurance activities, chart reviews, and corrective-action processes.
- Ensure services are delivered in a person-centered manner that promotes individual choice, independence, dignity, community integration, and achievement of identified goals.
- Maintain confidentiality and ensure compliance with HIPAA, agency policies, Medicaid requirements, LME/MCO requirements, and applicable state regulations.
- Perform other QP-related duties necessary to ensure continuity, quality, and compliance of services.
Experience & Qualifications
- Must meet applicable North Carolina requirements to function as a Qualified Professional (QP).
- Bachelor's degree in a human services or related field with the qualifying professional experience required for the position.
- Previous experience working in a QP role is required.
- Previous hands-on experience using THERAP is required.
- Experience working with individuals with intellectual/developmental disabilities is required.
- Experience supporting individuals with co-occurring mental health, behavioral health, and/or substance-use needs is strongly preferred.
- Working knowledge of ISP/PCP development, implementation, monitoring, and documentation.
- Experience reviewing service documentation and providing oversight to direct support professionals and/or residential providers.
- Knowledge of incident management, reporting requirements, health and safety monitoring, and appropriate follow-up.
- Familiarity with North Carolina Medicaid-funded community services and LME/MCO systems is preferred.
- Experience participating in treatment-team meetings and communicating with guardians, care managers, providers, and other community partners.
- Supervisory or leadership experience within a community-based human services setting is preferred.
- Strong computer skills and ability to effectively manage electronic records and documentation.
Clinical licensure such as LCSW or LCMHC is not required for this position.

Transportation & Travel Requirements
This position includes regular community-based responsibilities and is not a fully remote position.
Candidates must:
- Possess a valid driver's license.
- Have access to a reliable personal vehicle.
- Maintain current automobile insurance.
- Be willing and able to travel throughout Greensboro and surrounding service areas.
- Travel to member residences, AFL homes, community locations, hospitals, meetings, and other service locations as required.
- Be available for scheduled member visits, site monitoring, treatment-team meetings, and occasional urgent situations requiring QP involvement.
