Role Description
This is a full-time h role for a Social Worker based in Geneva. The Social Worker will provide direct support and services to patients, conduct assessments, create and implement individualized care plans, and collaborate with families and interdisciplinary teams. Additional responsibilities include maintaining documentation, participating in care coordination, and connecting patients with community resources to support their ongoing recovery.
Qualifications
- Ability to conduct thorough assessments, develop care plans, and implement interventions tailored to individual needs
- Strong interpersonal, communication, and counseling skills to work effectively with patients, families, and interdisciplinary teams
- Proficiency in maintaining accurate and timely documentation and records as required
- Knowledge of community resources and experience in connecting patients with supportive programs
- Flexibility and organizational skills to manage both in-person and remote responsibilities
- Bachelor’s or Master’s degree in Social Work, Psychology, or a related field (Master's preferred)
- Active state licensure in Social Work (LSW or LISW) or equivalent
- Experience in mental health care or substance use disorder treatment is a plus
