What are the responsibilities and job description for the Hospice RN Case Manager position at Generation Healthcare?
At Generation Hospice, we believe every patient deserves comfort, dignity, and quality of life at the end of life. We're looking for a compassionate, organized Registered Nurse to serve as an RN Case Manager. You'll be the clinical point person for your patients, their families, and our interdisciplinary team.
What You'll Do
- Perform comprehensive assessments of patients' physical, emotional, psychosocial, and environmental needs
- Develop and update individualized care plans with patients, families, physicians, and the interdisciplinary team
- Provide skilled nursing care, including pain and symptom management, medication administration, wound care, and catheter care
- Coordinate services with hospice aides, social workers, chaplains, volunteers, and therapists
- Educate patients and caregivers on disease progression, comfort measures, medications, and what to expect at end of life
- Present patient updates and collaborate in Interdisciplinary Group (IDG) meetings
- Maintain accurate, timely documentation that supports hospice eligibility and complies with Medicare Conditions of Participation
- Visit patients in homes, nursing facilities, assisted living, and hospitals across our service area
- Take part in the on-call rotation, including some weekends and holidays
What We're Looking For
- Current, unrestricted Ohio RN license
- Graduate of an accredited school of nursing
- At least one year of clinical nursing experience
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment, clinical judgment, and time-management skills
- Comfort with electronic medical records
Preferred
- CHPN (Certified Hospice and Palliative Nurse) credential
- Hospice, home health, palliative, oncology, geriatric, or med-surg experience
- Case management experience

This role involves regular travel between patient locations, lifting up to 25 lbs frequently and up to 50 lbs with assistance, and exposure to emotionally demanding situations.
