Only 2 holiday on-call shifts per year

What Youll Do
Serve as the primary RN for your patient and family caseload
Complete assessments and determine hospice eligibility
Lead the admission process and develop individualized plans of care
Evaluate physical, emotional, social, and spiritual needs and coordinate interventions with the interdisciplinary team
Communicate with attending physicians, team members, and partner agencies to ensure seamless care
Conduct regular patient visits to reassess needs and update care plans
Perform clinical pronouncements of death and provide compassionate support to families
Maintain accurate, timely documentation, care plans, and progress notes
Supervise and document care provided by Home Health Aides per federal, state, and company guidelines
What You Bring
Graduate of an accredited registered nursing program
Current RN license in the state of practice
Current CPR and First Aid certification
Knowledge of quality assessment and assurance practices
Strong clinical judgment and ability to make independent decisions
Compassionate communication skills and the ability to support patients and families
Valid drivers license and reliable, insured vehicle
