JOB SUMMARY
The Referral Specialist plays a critical role in coordinating patient care by managing incoming referrals to ensure timely and appropriate access to services. This position serves as a liaison between patients, healthcare providers, and external organizations, maintaining communication, documentation, and follow-through throughout the referral process. The RS serves as a resource by utilizing resources established through Gellert Health policies and procedures to ensure delivery of professional care while contributing to the organization’s values of Kindness, Integrity & Quality.
- Verify referral requirements based on insurance guidelines, provider networks, and clinical appropriateness.
- Maintain detailed, up-to-date records of all referral activity, ensuring accurate data entry into electronic health records (EHR) and referral tracking systems.
- Monitor the progress of each referral, ensuring timely follow-up, scheduling, and coordination until the intake or consultation is complete.
- Serve as a point of contact for patients regarding their referral status, addressing questions and resolving barriers to care and conduct outreach to engage potential patients and educate about Gellert Health and the referral process.
- Communicate with providers, care coordinators, and external agencies to facilitate a smooth referral process.
- Ensure compliance with HIPAA and organizational policies in handling patient information.
- Identify and escalate issues or delays in the referral process to the appropriate supervisor.
- Foster and maintain strong relationships with partners, healthcare providers, and potential referral sources, ensuring clear, timely, and accurate communication between referring and receiving parties.
- Ensure the accurate and timely execution of clerical tasks, including data entry and phone support, to maintain efficient daily operations.
- Outreaching telephonically to a minimum of 50 potential patients a day to connect them with Gellert Health services.
- Understanding of physical and behavioral health services
- Experience working with culturally diverse, chronically homeless, and vulnerable individuals, in medical and behavioral health services (preferred).
- Ability to communicate effectively telephonically.
- Excellent written & verbal communication skills
- Comfortable working in a fast paced, sometimes stressful environment with a strong focus on customer service.
- Resourcefulness, close attention to detail, able to work independently and within a team.
- Ability to drive personal vehicle to engage with patients if needed.
- Bilingual – Spanish highly desirable not required.
REQUIREMENTS
- At least 21 years old
- 39 months clear Motor Vehicle Record
- Current Arizona driver’s license and Auto Insurance
- Ability to successfully pass a criminal background check and a drug screening. (Any convictions will be evaluated based upon the conviction in relation to the job requirements. A criminal conviction is not necessarily a deterrent to employment.)
- College degree and/or relevant work experience, e.g., patient acquisition or relationship management in healthcare or related field.
- Active AZ Fingerprint Clearance Card
- Flu Shot (waiver available with possible mask requirement)

PROBATIONARY PERIOD
This position includes a standard 90-day probationary period to assess performance and fit within the company. In certain cases, this period may be extended at the discretion of management to provide additional time for evaluation or training.
