Position: Health Navigator - Bi-Lingual Spanish
Salary: $45,000 - $50,000 (paid hourly)
An additional $1,500 may be applied to the starting salary after the training period and an assessment for Spanish speaking proficiency, with demonstrated ability to support Spanish speaking patients.
Bonus Program: A total of $1,500 paid out over 1 year. (6 months, 9 months, and 1 year)

Not all your patients will require you to transport them directly, however, this remains an important requirement of the job to be ready and willing to do so. For this reason, you must have a valid AZ driver's license, registration and appropriate level of auto insurance. The company provides a generous auto-allowance to be used towards fuel and maintenance.

What is Health Navigation?
Health Navigation is the process of advocating for and guiding a patient through the medical and behavioral health system. We provide one-on-one assistance with appointments, medications, and health literacy. The average case load is approximately 15-25 patients per Navigator, based on the level of care and service required for the patients.
Gellert Health provides our patients with holistic and patient-centered assistance to access the care they need, which may include finding a healthcare provider, attending medical appointments together, understanding insurance coverage, and helping the patients better manage their medications and chronic conditions. By providing targeted support and guidance, we help our patients access timely and quality care. The ultimate goal of our program is to help patients receive the best possible care, and to improve their well-being.
Navigator Role and Responsibilities:
- Schedule healthcare appointments with patient
- Attend appointments with patient
- Keep track of patient’s medications and refills
- Document all appointment and patient information in an Electronic Medical Record
- Relay information from one provider to the other for continuity of care
- Help to prevent duplicative services and testing
- Increase utilization of preventative services, decrease ED use
- Work day-to-day on patient goals
- Connect patient to SDOH providers
- Improve day-to-day safety of patients' living environment
- Transport patient to critical appointments in personal vehicle (auto allowance provided)
Training
Gellert Health provides a comprehensive training program. Each navigator undergoes 3 weeks of paid classroom and in-field training prior to navigating. Following training and job shadowing, the newly qualified Navigator will be ready to navigate patients successfully while embracing the company’s values of kindness, integrity and quality. The Classroom Training also emphasizes what is at the heart of what we do, a passion for helping others. The navigators receive ongoing continuing education and training on a weekly basis through individual and group supervision.
Requirements:
- Full-time position Monday- Friday (8am-5pm with a 1-hour lunch break)
- Bi-lingual Spanish speaking
- Associate's Degree or Bachelor's Degree in a related field; or High School / GED plus demonstrated relevant experience in medical or behavioral health field.
- 21 years old with a current Arizona’s driver’s license with 39 months clear MVR, a four-door car, and auto insurance.
- Able to drive patients to and from appointments in their personal car
- Ability to successfully pass a criminal background check.
- Ability to successfully pass a 10-panel drug test. This consists of testing negative for: Methadone, Methaqualone, Benzodiazepines, Barbiturates, PCP, Propoxyphene, Opiates, Amphetamines, Cocaine, Marijuana.
- Excellent oral and written communication skills, and proficient typing skills.

- CPR/First Aid certification
- TB Test/Flu Shot - Exceptions may be discussed

- Preferred - experience working with culturally diverse and vulnerable populations.

Pay: Salary Range of $45,000 to $50,000
