What are the responsibilities and job description for the Work Release Case Manager position at Gateway Rehabilitation Center?
Gateway Rehab’s Community Transition Center located in Braddock, PA. has an opportunity for a Full-time Work Release Case Manager who reviews and approves schedules and budget. Position includes mainly daylight hours, but may include evening, and weekend hours with potential holidays. Please note, the schedule is subject to change dependent on program needs.
Responsibilities:
- Reviews and approves residents’ weekly schedules and budgets.
- Assists residents in job readiness.
- Placement and maintenance, including life and job skills education and counseling.
- Coordinates community services efforts.
- Maintains good working relationships with employers and community service contacts.
- Performs case management duties.
- Facilitates groups, transports patients, and performs custody control duties as needed.
Knowledge, Skills, and Abilities:
- Good communications skills.
- Knowledge of the 12-step program preferred.
Basic Requirements:
- High school diploma or GED
Preferred Requirements:
- Bachelor’s degree
- Chemical dependency treatment and/or corrections programming experience.
Additional Requirements:
- Pass a PA Criminal Background Check
- Obtain PA Child Abuse and FBI Clearances
- Pass Drug Screen
- TB Test
- Favorable Driving Record
Work Conditions:
- Drug and Alcohol Rehabilitation Center
- Office-based environment
- May have some travel
