What are the responsibilities and job description for the Adult/Children's Mental Health Case Manager position at Gateway Community Services?

Preferred: Candidates with a current MHRT/C, or those who are eligible and willing to apply before scheduling an interview (guidance will be provided if needed).

**But relevant experience and education will also be taken into consideration.**

Requirement: Must be able to serve clients in surrounding towns.

Per Diem Case Manager
Job Type: Per Diem, with Full-time and Part-time options available
Weekly Hours: 20-40 hours per week
Pay: $31.50/hour for billable hours, $15.00/hour for documentation, meetings, and travel.

Who We’re Looking For:

  • Education & Certification: Bachelor’s degree in Social Work or a related field. Must have or be able to attain MHRT-C certification within a 90-day probationary period.
  • License: Valid driver’s license in good standing.
  • Passion for Community Service: Our case managers support clients in finding essential resources and achieving independence in all areas of life.

Our Case Management Program serves youth and adults with a mission to:

  • Enhance functioning in home, school, and community settings
  • Empower individuals to reach their full potential

Your Role:

  • Conduct initial assessments to understand the needs and strengths of each client
  • Develop individualized treatment plans and work hand-in-hand with clients and families to achieve set goals
  • Advocate and connect clients to essential services: mental health, medical care, education, housing, and more
  • Collaborate with providers to ensure effective support and crisis management
