POSITION SUMMARY

The Targeted Case Managerment Adult Mental Health Case Manager provides community-based case management services under MaineCare Section 13. . The primary job of a Section 13 case manager is to help individuals access necessary medical, social, educational, and other services. They act as a central guide to ensure clients receive comprehensive, coordinated support in their communities.

ESSENTIAL RESPONSIBILITIES

• Conduct comprehensive assessments and gather collateral information
• Develop and maintain individualized service plans (ISP)
• Coordinate care across mental health, medical, housing, and social services
• Provide services in community-based settings (home, shelter, community)
• Monitor client progress and adjust services accordingly
• Respond to crisis situations and coordinate appropriate interventions
• Maintain timely, accurate, and compliant documentation
• Advocate for client access to services and supports
• Participate in supervision and interdisciplinary collaboration

QUALIFICATIONS

Required:

• BA or BS in the Humanities or Social Service.

Experience working Section 13 TCM Adults with intellectual disabilities and autism.

• Valid driver's license and reliable transportation

• Maintain caseload of 12-15 clients
• Meet productivity expectations
• Complete documentation within 24 hours (required)

WORK ENVIRONMENT

This is a community-based position requiring travel throughout the Portland area. Work occurs in client homes, community settings, and office environments.
