Position Overview


We are seeking a detail-oriented and compassionate Medical Assistant to support our Chronic Care Management (CCM) program. This role focuses on coordinating care for patients with chronic conditions, ensuring continuity of care, and improving patient outcomes through consistent communication and follow-up.

Key Responsibilities

• Conduct monthly CCM outreach calls to eligible patients
• Document patient interactions accurately in the EMR system
• Develop and update patient care plans under provider supervision
• Coordinate referrals, labs, and follow-up appointments
• Monitor patient compliance with medications and treatment plans
• Collaborate with providers and clinical staff to ensure high-quality care
• Assist with identifying patients eligible for CCM services
• Ensure compliance with CMS guidelines and documentation requirements

Qualifications

• Certified Medical Assistant (CMA) or equivalent experience
• Minimum 1 year of experience in a clinical or outpatient setting
• Experience with Chronic Care Management (CCM) preferred
• Strong communication and patient engagement skills
• Proficiency in EMR systems (e.g., eClinicalWorks, gMed, Epic, etc.)
• Ability to multitask and manage time efficiently

Preferred Skills

• Bilingual (English/Spanish) a must
• Experience with Medicare patients and care coordination
• Knowledge of CMS CCM billing guidelines
