Job Summary
We are seeking a dedicated and compassionate Qualified Medication Administration Personnel (QMAP) to join our team for the Thursday through Saturday shift, 6:00 AM to 6:00 PM. Your expertise will support individuals with diverse needs, including those requiring memory care, hospice support, or assistance with daily activities. Responsibilities
- Administer medications accurately following prescribed protocols and document all medication activities in electronic health record (EHR) systems such as Epic or Cerner.
- Monitor residents’ vital signs, assess physical and emotional needs, and report any changes to the healthcare team promptly.
- Assist with activities of daily living, including mobility, personal hygiene, and feeding, ensuring safety during manual handling and heavy lifting tasks.
- Maintain compliance with HIPAA regulations and state healthcare laws while safeguarding patient confidentiality through proper medical documentation.
- Support residents with specialized care needs such as Alzheimer’s, dementia, or memory care by implementing personalized care plans.
- Collaborate with nursing staff and other healthcare professionals to coordinate comprehensive patient care in accordance with medical terminology and physiology knowledge.
- Adhere to all safety protocols including first aid, CPR certification, catheter care, tube feeding procedures, and basic life support standards.
Experience
- Prior experience working in assisted living facilities, nursing homes, hospitals, or home care environments is highly preferred.
- Demonstrated knowledge of medical records management using EMR/EHR systems like Epic or Cerner.
- Experience caring for geriatric patients with conditions such as Alzheimer’s disease or other memory impairments.
- Familiarity with state Board of Nursing requirements and healthcare regulations relevant to senior and hospice care.
- Background in caregiving roles involving medication administration, vital signs monitoring, medical documentation, and working with individuals with disabilities.
- Ability to perform heavy lifting safely while maintaining proper manual handling techniques.
- Strong understanding of medical terminology, anatomy knowledge, and physiology related to patient care needs.
