Responsibilities
- Administer medications and assist with medication management following prescribed protocols, ensuring compliance with HIPAA regulations and medical documentation standards.
- Provide personal care assistance including hygiene, mobility support, feeding (including tube feeding), catheter care, and vital signs monitoring.
- Support residents with activities of daily living in assisted living, nursing homes, or home care settings while maintaining dignity and respect.
- Document all patient interactions accurately using EMR (Electronic Medical Record) systems such as Epic or Cerner, ensuring detailed medical records are maintained.
- Assist with specialized care needs such as Alzheimer’s/dementia/memory care and hospice support, adapting approaches to meet individual needs.
- Collaborate with healthcare teams to monitor physiological functions and report any concerns related to anatomy or physiology knowledge.
- Follow infection control protocols and safety procedures to ensure a clean, safe environment for residents and staff.
Experience
- Prior experience working in assisted living communities, nursing homes, hospitals, or home health environments is highly preferred.
- Demonstrated knowledge of medical terminology, medication administration procedures, and medical documentation practices.
- Experience working with individuals with developmental disabilities or disabilities-related caregiving is a plus.
- Familiarity with EMR/EHR systems such as Epic or Cerner enhances efficiency in documentation and patient record management.
- Background in acute or chronic care settings that involve Alzheimer’s/dementia/memory care or hospice services is advantageous.
- Strong understanding of vital signs measurement, anatomy, physiology, catheter care, tube feeding techniques, and medication protocols is essential.
- Certification as a QMAP (Qualified Medication Assistance Professional) is required; additional certifications in caregiving or nursing are beneficial.
