Qualifications:

· Must be 18 years of age or older

· Must meet all insurance criteria with submission of driver’s license for record check

· Must complete a physical examination and a criminal record check.

Responsibilities:

· Care of all individuals – including but not limited to assisting individuals in the restroom, assisting with lunchtime (this can include feeding), dietary needs, etc.

· Supervision of all individual’s activities and community outings as per their individual support plan.

· Implementation of Behavior Support Plans and/or SEEN plans

· Assisting with cleaning of the building

· Meeting all ODP regulations

· Following all Ganister Station ADS policies and procedures/handbook

· Adhering and following all Incident Management requirements (mandating reporting)

· Adhering to all HIPPA privacy guidelines

· Administer of Medication (Ganister Station will complete training for medication administration)

· Completion of all necessary documentation

· Transport individuals for pick-up and drop-off as well in the community

· Maintaining all vehicle and mileage logs
