POSITION OVERVIEW

The Hospice Admissions Registered Nurse serves as the initial clinical contact for prospective hospice patients and families in the Dallas–Fort Worth area.

Responsibilities include evaluating patients for hospice eligibility, explaining services and benefits, completing admissions, and creating personalized care plans for a seamless transition into hospice care.

Collaboration with clinical leadership, physicians, and the interdisciplinary team is key to ensure a compassionate and coordinated admission experience for every patient.

This position reports to the Clinical Manager and receives guidance from the Chief Operating Officer.

SERVICE TERRITORY

Primary service areas include:

• Collin County

• Dallas County

• Denton County

• Tarrant County

• Cooke County

Regular local travel within the assigned Dallas–Fort Worth region is required.

KEY RESPONSIBILITIES

• Conduct assessments to determine hospice eligibility for prospective patients.

• Review medical history, symptoms, and clinical documentation.

• Communicate with physicians, patients, families, and caregivers throughout admissions.

• Explain hospice services, benefits, and care expectations with compassion.

• Facilitate difficult conversations with empathy and respect.

• Complete admission assessments, consents, and documentation accurately and promptly.

• Develop tailored care plans based on patients’ needs and goals.

• Coordinate necessary medications, equipment, and services for smooth transitions.

• Ensure seamless onboarding of new patients with the interdisciplinary team.

• Document clinical findings and changes in condition promptly.

• Maintain accurate records in compliance with regulations.

• Manage daily admission activities independently while communicating with leadership.

• Participate in on-call duties as required.

• Assist with additional clinical or administrative tasks as needed.

IDEAL CANDIDATE

The ideal candidate will have:

• Minimum one year of hospice nursing experience.

• Deep understanding of hospice philosophy, Medicare criteria, and benefits.

• Compassion for patients facing life-limiting illnesses.

• Strong communication and critical-thinking skills.

• Ability to build trust quickly in sensitive situations.

• Excellent organization, documentation, and time-management abilities.

• Experience in interdisciplinary teamwork.

• Independence to manage schedule effectively.

• Flexibility in handling changes in patient needs.

• Professionalism and patient-centered approach.

• Proficiency in electronic medical records and Consolo is a plus.

REQUIRED QUALIFICATIONS

• Active Texas RN license.

• Minimum one year of hospice nursing experience.

• Knowledge of hospice eligibility and admission process.

• Valid driver’s license, personal transportation, and auto insurance.

• Willingness to travel within Dallas–Fort Worth area.

• Availability for on-call responsibilities.

SCHEDULE

• Full-time with flexibility based on patient needs.

• Limited on-call duties as required.

• Local travel within Collin, Dallas, Denton, Tarrant, and Cooke counties.

COMPENSATION

• Base salary of $85,000–$90,000 per year.

• Mileage reimbursement and cell phone allowance.
