Futures Essentials Inc. is seeking a qualified Nutritionist to support Social Care Network (SCN) programming by conducting comprehensive nutrition intake assessments and providing evidence-based health education to individuals and families. The Nutritionist will play a critical role in identifying nutrition-related social needs, supporting food security initiatives, and promoting preventative health education through weekly health tips and community outreach materials. This role supports Medicaid-aligned social care services and essential needs interventions.
Key Responsibilities SCN Intake & Nutrition Assessments
- Conduct comprehensive nutrition intake assessments for individuals enrolled in SCN services.
- Evaluate dietary habits, food access, cultural food practices, and health-related nutritional risk factors.
- Identify nutrition-related social determinants of health (e.g., food insecurity, limited access to healthy foods).
- Develop individualized nutrition support plans aligned with SCN service guidelines.
- Collaborate with care managers, navigators, and case workers to coordinate services.
- Document assessments and interventions in accordance with SCN documentation and compliance requirements.
- Participate in interdisciplinary case reviews when appropriate.
Health Education & Weekly Content Development
- Develop and distribute weekly health and nutrition tips for clients and the broader community.
- Create culturally sensitive, easy-to-understand educational materials.
- Provide practical guidance on:
- Budget-friendly healthy eating
- Meal planning
- Chronic disease prevention (diabetes, hypertension, obesity)
- Heart-healthy and low-sodium diets
- Nutrition for children and families
- Contribute content for social media platforms and newsletters.
- Support educational workshops and community presentations when needed.
Community & Program Support
- Assist with nutrition-related initiatives connected to essential needs distribution.
- Support partnerships with food pantries, community kitchens, and local providers.
- Provide group nutrition education sessions at community events or partner sites.
- Collaborate with faith-based organizations and community partners to promote healthy lifestyle practices.
Documentation & Compliance
- Maintain accurate, timely documentation of all nutrition assessments and services provided.
- Ensure services align with SCN program requirements and Medicaid-compliant standards.
- Track client engagement and outcomes related to nutrition interventions.
- Assist with reporting requirements for grants and network partners.
Qualifications & Education
- Bachelor’s or Master’s Degree in Nutrition, Dietetics, Public Health Nutrition, or related field.
- Registered Dietitian (RD) or Certified Nutritionist preferred (as required by state regulations).
Pay: $25.00 - $30.00 per hour
