POSITION: Case Manager
SUPERVISED BY: Clinical Director
INTERRELATIONSHIPS:
Maintains open communication lines with Clinical Director center staff, attending clinicians and physicians, patients and families.
POSITION PURPOSE
Our Case Manager's responsibility consist of being part of the treatment team meeting, completing case management portion intakes, developing discharge plans for all clients, completion of FMLA or disability, return to work letters, assistance with legal matters, Facilitating groups within your scope of practice, and relaying insightful information to the clients primary therapist.
POSITION REQUIREMENTS/QUALIFICATIONS
1. EDUCATION:
Associates’ Degree or higher
2. EXPERIENCE:
At least two years of experience in a substance treatment or health care setting.
3. LICENSURE/CERTIFICATION:
CADC Certificate is Preferred. CADC Interns with experience are welcome to apply as well.
4. SKILLS/ABILITIES:
Ability to work as a team member and have management, communication, organizational and interpersonal skills.
- Ability to work under stressful conditions in a fast paced environment while being flexible in relation to department needs.
- Completion FMLA, Short Disability and Social Security
- Basic Knowledge of Joint Commission Standards.
- Demonstrates Proficiency in Verbal & Written Communication Skills.
- Knowledge of State & Federal Statutes Regarding Patient Confidentiality Laws.
- Knowledge of Drug-Free Workplace Policies.
- Knowledge of Workplace Violence.
- Knowledge of Corporate Integrity & Compliance Program.
- Knowledge of current utilized technology.
- Experience with clinical documentation in an EMR system like KIPU.

2. PERFORMANCE/PROFICIENCY STANDARDS
Performance standards are integrated within the position responsibilities. The principle functions of the position identified shall not be considered as a complete description of the all the work requirements and expectations that may be inherent in the position.
SPECIFIC AREAS OF RESPONSIBILITY TO POSITION
1. Answers any questions callers may have that are non-clinical in nature; appropriately transfers calls as needed.
2. Completes intake assessments and designated intake portion of client’s charts.
3. Assists in developing a discharge plan for clients and communicating that plan with the clinical team in the treatment team meetings.
4. Provide accurate information to the billing company or third party payors as assigned.
5. Flagging for signatures needed on patient documentation (when needed)
6. Work independently and able to handle frequent interruptions.
7. Must have good organizational skills.
8. Must have good interpersonal skills.
9. Assists co-workers as assigned by the Clinical Director
10. Attend monthly staff meetings, and communicates daily with facility staff and administration as assigned by Clinical Director.
11. Performs duties independently, talking a proactive approach to problem situations, and seeking supervision on an as needed basis.
12. Demonstrates objectivity and maturity under stress and responds positively to changing situations in the workplace.
13. Performs in a professional manner and role- models a positive image of mental health.
14. Communicates effectively with staff, independent contractors and vendors.
15. Understands and adheres to legal guidelines pertaining to patient confidentiality and patient rights.
16. Performs other duties as assigned.
Job Type: Full-time
Pay: $40,000.00 - $50,000.00 per year
