Frontpoint Health is a fast-growing home health agency committed to delivering personalized, high-quality care. We're currently seeking experienced and compassionate Physical Therapists (PTs) to join our team and help patients recover safely and comfortably in their homes.
What You'll Do
- Provide in-home physical therapy to patients recovering from injury, surgery, or illness
- Evaluate patients' physical conditions and develop individualized care plans
- Implement therapeutic exercises and mobility techniques
- Educate patients and caregivers on rehabilitation best practices
- Collaborate with nurses, physicians, and care teams
- Complete timely and accurate documentation in our EMR (Kinnser/WellSky)
Requirements:
- Active Texas Physical Therapist (PT) license (Required)
- 1 year of home health experience (Required)
- Reliable transportation and valid driver's license (Required)
- Familiarity with Kinnser/WellSky (Preferred)
- Excellent time management and communication skills
- CPR certification (Preferred)
Compensation
- Sign-On Bonus: $15,000 for eligible full-time candidates
