Hospice Aide – Job Description
Job Summary: The Hospice Aide provides personal healthcare and related supportive services to patients in their place of residence. Responsibilities include assisting with activities of daily living, promoting a clean and safe environment, observing and reporting changes in the patient’s condition, and working cooperatively with the interdisciplinary hospice team. All care is provided under the direction and supervision of a Registered Nurse.
Job Qualifications
Education
Candidate must meet one of the following requirements:
- Completion of an approved Aide Training/Competency Evaluation Program as required by Medicare (§418.76).
- OR successful completion of a state‑approved competency evaluation program meeting federal requirements.
- OR completion of Nurse Aide training/competency evaluation approved by the state and listed on the state nurse aide registry.
- OR completion of a state licensure program meeting requirement of §418.76(b).
- High School Diploma preferred.
Licensure
- Must have a valid driver's license
Skills
- Ability to work with patients, families, visitors, and interdisciplinary team members in a compassionate, professional manner.
- Strong communication skills, observation skills, and the ability to report changes promptly.
- Ability to follow the hospice plan of care and complete tasks efficiently.
- Successful completion of required skills and competencies.
Experience
- At least one year of experience working as a Hospice Aide or Nurse Aide in a hospital, nursing home, home health agency, hospice, or long‑term care facility.
- Completion of a state-approved Certified Nursing Assistant or Certified Home Health Aide training
Transportation
- Reliable transportation and valid auto liability insurance required.
Environmental & Working Conditions
- Work is performed in patient homes, facilities, and community settings.
- Possible exposure to: infectious diseases, blood/body fluids, chemicals, and hazardous materials.
- Potential for injury from lifting or transferring patients.
- Exposure to various weather conditions during travel.
Physical Requirements
- Standing, walking, bending, and lifting up to 50 pounds.
- Must be able to meet physical and mental demands of caring for seriously ill patients and participating in family‑centered care.
- Requires stress tolerance and the ability to adapt to rapidly changing situations.
Essential Job Functions
Patient Care
- Provide direct personal care to the patient under the direction of the RN and in alignment with the Aide Plan of Care.
- Assist with safe transfers, mobility, and ambulation following hospice policy.
- Provide bathing, grooming, skin care, toileting, oral care, nail care, and other hygiene assistance.
- Maintain a clean, safe environment, including light housekeeping tasks related to patient care.
- Support nutritional needs, including meal preparation and feeding assistance when required.
Communication & Documentation
- Document all care provided according to hospice policy and regulatory requirements.
- Observe and report changes in the patient’s condition, appetite, behavior, skin integrity, and functional ability.
- Communicate effectively with the hospice team, including timely updates to the RN.
Professional & Interpersonal Responsibilities
- Protect patient dignity, privacy, and confidentiality.
- Demonstrate respectful communication with patients, family members, and caregivers.
- Participate in in‑services, training, and competency evaluations as required.
- Promote and practice infection control and universal precautions.
Safety & Compliance
- Follow the Hospice Plan of Care, organizational policies, and regulatory requirements.
- Adhere to proper body mechanics and safe patient‑handling procedures.
- Comply with OSHA, HIPAA, CMS Conditions of Participation, and state regulations.
Organization & Philosophy

- Promote the hospice philosophy of comfort‑based, person‑centered care.
