Essential Functions
- Provide skilled nursing services in accordance with the individualized plan of care.
- Perform ongoing patient assessments (physical, psychosocial, and environmental) and document findings.
- Monitor patient condition and communicate changes to the RN Case Manager and physician.
- Administer medications and treatments, ensuring safe and effective medication management.
- Implement physician orders and nursing interventions.
- Support pain management and educate patients and families on care plans and disease processes.
- Coordinate care with the interdisciplinary team to ensure continuity and quality outcomes.
- Complete timely, accurate clinical documentation, including OASIS (if applicable).
- Ensure compliance with Medicare guidelines, CHAP standards, and agency policies.
- Participate in discharge planning, care transitions, and referrals to community resources.
- Attend case conferences, in-services, and quality improvement (QAPI) initiatives.
- Active, unrestricted RN license.
- Graduate of an accredited nursing program.
- Minimum of 1 year of RN experience (home health preferred).
- Current CPR certification, TB screening and required health documentation.
- Valid driver's license, reliable transportation and auto insurance.
- Maintain HIPAA compliance and patient confidentiality.
- Adhere to Agency policies, ethical standards and professional appearance.
- Complete visits, documentation, and tasks accurately and within 48 hours of the visit.
- Participate in ongoing training and continuing education.
